Terms & Conditions
Eligible purchases are cumulative annually. Invoices for the prior year must be submitted by January 31 of the following year to qualify. Multiple invoices can be submitted at one time. Invoices can be submitted at any time throughout the year. No purchase orders or order confirmations will be accepted. Allow 10 business days for processing following submission of form. There is no limit to the number of users that can get access within a qualifying period.
The person who submits the form will receive user access codes earned by the invoice(s) submitted. It is their responsibility to distribute the access codes within their clinic. By filling out the form, this person agrees that they are acting on behalf of their clinic and are authorized to receive the access codes.
Products must be purchased and billed during the program term from a Parnell distributor partner.
